The Red Potion was first introduced into Terraria in order to test how debuffs work on players, which explains its extremely debilitating effects. If players consume a Red Potion, they will receive 11 different debuffs for a lengthy period of time (1 hour in Classic Mode, 2 hours in Expert Mode, and 3 hours in Master Mode).
Afterward, however, they were changed a bit for the sake of a seed-related secret. These usually debilitating potions will instead give players 3 mostly random buffs, but only if they are consumed in a “For the Worthy” world.
What Red Potions Do
A Red Potion will cause different effects depending on where one drinks it–in a regular world, it will cause players to be afflicted with 11 debuffs simultaneously. Here are those effects and what they do:

-4 defense -5. 1% melee damage -5. 1% melee attack speed -10% movement speed
When consumed in a “For the Worthy” world, however, Red Potions will randomly apply 3 buffs. The randomization can select the same effect multiple times. Here is a list of the possible effects that can be acquired:

How To Get Red Potions
To get Red Potions, players will need to find them in chests or get them as drops from specific enemies. However, these can only be done in worlds generated with specific seeds.
Getting Red Potions In Drunk World
This twist on regular worlds can be found with the following seeds:

In this world, there is a 3.33% chance to find a Red Potion in a chest.

These worlds created to commemorate the 10th anniversary of Terraria can be found with the following seeds:

Differently from the other special worlds, the only way to get Red Potions here is to find them as drops from Jungle Mimics.
Getting Red Potions From For The Worthy Worlds
In these kinds of worlds, there is a 10% chance for a Red Potion to appear from a chest.
How To Enter A “For The Worthy” World
This map is quite easy to find, as, in the seed text field, players must enter “for the worthy” (not case sensitive) to generate this specific universe. For the Worthy is a world where enemies are significantly more difficult to defeat. Here’s a list of the main differences between this super-tough world and one generated with a random seed:
